| Abstract: | Problematika efektivní péče o osoby s nehojící se ránou je složitou interdisciplinární záležitostí. Práce se zabývá moderními postupy v léčbě chronických ran a defektů. Konkrétně metodami nazývanými souhrnně "metody vlhkého hojení". Týká se širokého záběru pacientů ve všech medicínských oborech a věkových kategoriích. Teoretická část práce se zabývá chronickou ránou, její definicí, klasifikací, hojením, ovlivňujícími faktory i hodnocením a dokumentací. Dále teoretická část stručně popisuje a třídí metody i prostředky moderní terapie. V praktické části je podrobně zpracována kazuistika pacienta s chronickými defekty a dalšími přidruženými onemocněními. Je popsán ošetřovatelský proces, který je zaměřen na postup, metody a prostředky hojení defektů. Jsou zde určeny i další ošetřovatelské problémy přidružených onemocnění, které hojení i celkový stav ovlivňují a komplikují. Je popsáno stanovení ošetřovatelských procesů a nezbytných edukačních plánů. Vlastní hojení defektů je doloženo fotodokumentací v přílohách. V diskuzi je komentováno dosažení cíle bakalářské práce. Pomocí kazuistiky byly prokázány účinky této metody a to, jaké výhody oproti klasickým metodám přinesly pro pacienta, pro ošetřující personál a potažmo i pro nemocnici. |
| Abstract in different language: | The issue of effective care for people with non-healing wound is a complex interdisciplinary matter. The work deals with modern methods in the wound management, in the treatment of chronic wounds and defects. Specifically methods collectively called "methods of moist healing". This applies to a broad scope of patients in all medical disciplines and age categories. The theoretical part is dealing with a chronic wound, its definition, classification, healing, the influencing factors and the evaluation and documentation. Furthermore, the theoretical part briefly describes and classifies the methods and means of modern therapies. In the practical part are elaborated patients with chronic defects and other associated diseases. There is described the nursing process in the practical part, which focuses on the process, methods and means of defect-healing. There are also determined the other nursing problems associated diseases that influence and complicate healing and the overall condition. It is also described the determination of nursing plans and the necessary educational plans. Custom healing of defects is documented in photographs annexes. The achieving of the thesis goals is commented in the discussion. By using case studies and casuistry were demonstrated the effects of this method and also the advantages for patients, nursing staff, and last but not least for hospital over traditional methods. |
